What is the Deep Plane Facelift? 

Traditional facelifts usually only elevate and reposition the skin and the Superficial Musculoaponeurotic System (SMAS). The Deep Plane Facelift addresses the tissues deep to the SMAS that are the true causes of facial aging. A deep plane facelift addresses the deeper tissues that cause facial aging by repositioning, tightening, or releasing the underlying muscles, fat pads, and retaining ligaments than traditional facelifts do not address to achieve a more successful rejuvenation of the face.  Therefore, the deep plane facelift lasts longer than other types of facelifts, providing a youthful, natural rejuvenation for 15-20 plus years.

Is a Deep Plane Facelift Right for Me?

What Sets the Admire MD® Deep Plane Technique Apart

You will go under general anesthesia for approximately 3 to 3.5 hours.  The incisions are created by beveling the blade to save all the hair follicles, thereby reducing the risk for alopecia (hair loss), and allow for imperceptible scars in the hairline and around the ears.  There is a small incision under the chin for the deep plane neck lift portion. Using these incisions, Dr. Admire will elevate, reposition and tighten the underlying deeper tissues, including the muscles and fat pads deep to the SMAS. The buccal fat pad may be partially removed.

He will then place permanent suspension sutures to elevate and resuspend the SMAS in a superior vector. Then the key retaining ligaments are released to elevate and reposition the skin independently.  The incisions are then closed free of tension.  The deep plane neck lift in Scottsdale elevates, repositions, and partially removes the muscles, fat, and submandibular glands.  The platysma is then tightened with a combination of permanent and dissolvable sutures.  A drain is placed in the neck and removed in 2-4 days.

What is the Deep Plane Neck Lift? 

Traditional neck lifts usually only elevate, reposition, and tighten the superficial neck muscles called the platysma. The platysma muscles are an extension of the SMAS in the face.  The superficial fat layer is then usually removed directly or by liposuction.  The Deep Plane Necklift addresses the tissues deep to the SMAS that are the true causes of aging in the neck. These structures include the submandibular glands, deeper neck muscles, and deep fat layers. The Deep Plane Necklift removes all or part of these structures to dramatically sculpt and contour the neck in a way that traditional neck lifts can not achieve.  The Deep Plane Necklift lasts longer than other types of neck lifts, providing a youthful, natural rejuvenation lasting for 15-20 plus years.

How Does the Deep Plane Facelift Achieve a Greater Rejuvenation?

The areas targeted are different

The skin and underlying soft tissues sag in the face medially from a landmark that begins at the angle of the jaw and ends at the lateral corner of the eye. The skin lateral to this line stays relatively firm by adhering to the parotid gland just in front of the ear.

The deep plane facelift addresses a more medial and deeper area than any other facelift, extending towards the mouth and jowls. Therefore, the most aged areas of the face - the deep nasolabial folds, marionette lines, and jowls are targeted, resulting in more significant, rejuvenated changes. 

The release of ligaments

The deep plane facelift releases four ligaments that anchor muscle and connective tissue to the skin. Releasing these ligaments allows the deeper tissues of the face to be fully released and resuspended superiorly, resulting in a dramatic, but the natural improvement that is uniquely different from any other facelift. The release of the ligaments is an extended technique of the deep plane facelift.

Deep Plane Facelift Recovery

A deep plane facelift allows for a shorter recovery time because there is less skin and SMAS trauma during this procedure compared to traditional facelifts.  The permanent sutures and deeper suspension does not rely on skin stretch or over-pulling of the skin.  The facial and neck wrap placed during surgery is removed 2-3 days later.  Most patients can return to work or light activity in about 1-2 weeks. Full activity can be expected in 3-4 weeks.

Recovery Timeline

The first few days after your procedure

When your procedure is complete, you will most likely be able to recover at home the same day. Walking is encouraged to keep the circulation going to promote healing.

  • No ice or cold packs are needed, unless this is combined with an eyelid procedure. 
  • The head is mildly elevated for the first week, and a neck roll is used when sitting or laying down.
  • A single drain will be removed within 2-4 days. 
  • Once the drain is removed, you may shower and wash your hair.
  • The sutures are removed in 7 days. 
  • A chin strap is worn for the first 7 days and discontinued when the sutures are removed.
  • There may be some weakness of the lip and smile muscles.

1-2 weeks 

  • Light physical activity is permitted.
  • Return to work is expected, including driving. 
  • Swelling will begin to ease.
  • Makeup may be applied after seven days, or when the sutures are removed.
  • May resume normal skincare products. Dr. Admire will recommend a medical-grade skin care regimen to maintain the tight youthful looking skin from the a deep plane facelift
  • Continue to keep your head elevated while sleeping.

2-3 weeks 

  • Significant improvement in bruising and swelling is expected.
  • The face will continue to “soften,” as there will be less of a pulled look, especially around the mouth.
  • Most patients are comfortable with resuming all work and general activities.
  • Wait until the third week passes to resume moderate exercise.

4-6 weeks 

  • Nearly all patients can resume any type of strenuous, physical activity and exercise.
  • The swelling, especially in the upper cheeks will have nearly subsided and the youthful, younger appearance will become clearly evident.
